Step 1:
Download and complete the appropriate business entity forms from the California Secretary of State website.
Step 2:
Download and complete our SOS Business Entity Filing Order Form.
Optional: Download, complete, and sign our Credit Card Authorization Form.
Step 3:
Mail your Completed Order Form, the California SOS Business Entity form(s), and the payment method (ie. Check or Credit Card) to our office in Sacramento:
